Icons dissappear when using layout

I'm using a layout to have the same ribbon in all the screens, but after placing icons and going to the browser, some icons disappear. It happens without a pattern, sometimes and not all of them.

What could it be?

Check if you have plan links pointing to empty plans. Such plans are not rendered so plan links elements are not shown.
